Supplementary Information Form for admission to a school with a religious character

When applying for a place at a school with a religious character it is necessary to complete SIF, your parish priest is no longer required to sign it.  To obtain a paper copy of the SIF please call the school office on 01252 548123 or click the link below for a PDF copy:



Supplementary Information Form

This form must be completed and returned to the school with your child’s baptismal certificate. If a baptismal certificate is not available a letter from your religious leader, on headed paper, can be submitted.  If you are not able to provide either of these it is important you still complete and return the SIF.

  • The published admission number (PAN) is 30 places since September 2020. 

In-Year Applications


An application can be made for a place for a child at any time outside the admission round and the child will be admitted where there are available places.  An application should be made to the school by completing the in-year admissions application form at and completing a Supplementary Information Form (part of the admissions arrangements) and returning it to Mrs B Dias via the school office.


Where there are places available but more applications than places, the published oversubscription criteria, as set out in the admission arrangements for 2021-2022, will be applied Parents are advised to read the admission arrangements carefully before making their application.


If there are no places available, the child will be added to the waiting list.  Please see the admission arrangements for more details.


You will be advised of the outcome of your application in writing as soon as possible.  Applicants must be informed of the outcome of their application within 15 school days of receipt, but the aim is to notify applicants of the outcome of their application within 10 school days of receipt.


You have the right to appeal to an independent appeal panel if your application is unsuccessful.
